What the buyer is requesting
Details
Resources Safety and Health Queensland (RSHQ) is seeking to engage a suitably qualified expert/s to undertake a clinical audit of health assessments under the Coal Mine Workers’ Health Scheme (CMWHS). This clinical audit will assess the quality and consistency of clinical decision making, specifically related to causality decisions in diagnosis of chronic obstructive pulmonary disease (COPD) and broader matters related to the respiratory health surveillance component of the health assessment.
Background information / Compatibility requirements
Details
The Coal Mining Safety and Health Act 1999 (Act) and the Coal Mining Safety and Health Regulation 2017 (Regulation) establishes a regulatory framework that protects the safety and health of workers affected by operations at a coal mine in Queensland. This includes provisions for mandatory health surveillance through the Coal Mine Workers’ Health Scheme (CMWHS). Coal mine workers must have an assessment on entering the industry and then at least every five years while employed in the industry. The CMWHS is used to determine fitness for work and to provide early diagnosis and intervention for respiratory diseases like coal workers’ pneumoconiosis and silicosis.

RSHQ maintain a register of providers approved to carry out functions under the CMWHS. This includes examining medical officers (EMO) who complete the examination section of the health assessment and supervising doctors who carry out, supervise and report on health assessments. Employers are required to appoint one or more supervising doctors as their appointed medical adviser (AMA). A list of approved providers registered under the CMWHS is published on the Register of doctors and medical providers | Business Queensland website.

Quality health assessments are vital for accurate and timely disease detection, ensuring suitable fitness for work decisions and identifying workplace restrictions to manage risks to acceptable levels. Recent compliance activities and anecdotal evidence highlights the need for an evaluation of completed health assessments to ensure quality, reliability and consistency in decision making.

Consistency and comparability, including criteria that yield consistent outcomes across reviews, readiness for blinded review and clear rationale when departing from CPG or Return to work guideline.

Question:
Good afternoon

Is there an existing or previous audit against which the outcomes of this audit are intended to be compared? If so, will the previous audit tool and/or questions be made available to ensure consistency in data collection and comparability of results?

Thank youl

Please refer to attached document 'Q&A - Doctor Quality Assessments - COPD and Health Assessment Review - RSHQ.2026.117.2'.

Question:
1. Could you please confirm the whether the CMWHS assessments which are to be reviewed as part of the audit will be de-identified with respect to both the patient (worker) and the doctors who have conducted and signed off on the medicals?

2. Does RSHQ have a target number of assessments to reviewed/audited, and/or a target time period, or will the supplier determine an appropriate number of assessments to review?

3. Noting the item regarding “grossly inadequate, incompetent or reckless conduct that could expose workers to an unacceptable level of risk”, does RSHQ have a definition or guide as to what they consider would characterise type of conduct?

4. Regarding the clinical review of COPD cases, does RSHQ have a target number of assessments to reviewed/audited and/or a target time period, or will the supplier determine an appropriate number of COPD cases to review?

5. The tender advises that the review is a clinical audit. Is there an expectation that the supplier will need to be involved in the full audit cycle including reviewing of any recommendations at a later stage (subsequent to providing the report)?

6. Regarding the clinical review of COPD cases, could RSHQ confirm whether any recommendations made by the supplier after review of individual COPD cases, will be used to reconsider previous liability decisions which have been made about the work-relatedness (or non-work-relatedness) of individual COPD cases?
